These terms apply to onformphysio.com.au, operated by On Form Physiotherapy (ABN 86 493 483 779). By using this website you agree to them. They sit alongside our Privacy Policy, which explains how we handle your personal and health information.

1. The information here is general

Everything on this website — service pages, blog articles, exercise descriptions, FAQs — is general information about physiotherapy, bike fitting and related services. It is not individual clinical advice, and it cannot take into account your history, your diagnosis or your circumstances.

Do not rely on this website in place of an assessment by a physiotherapist, your GP or another qualified health practitioner. If you have symptoms, get them assessed. Do not delay seeking care, or disregard advice you have already been given, because of something you read here.

2. No practitioner–patient relationship

Reading this website, sending an enquiry through our contact form, or messaging us on social media does not create a practitioner–patient relationship. That begins when you attend a consultation and we complete an assessment with you.

3. Responses to treatment differ

People respond to physiotherapy and to changes in bike or running position differently, depending on their condition, history, training load and goals. Nothing on this website is a promise or guarantee of any particular result.

4. In an emergency

This website is not monitored for urgent matters. If you are seriously injured or unwell, call 000 or go to your nearest emergency department.

5. Bookings, fees and cancellations

Appointments are booked through our online booking platform, which is operated by a third party under its own terms and privacy policy. Fees are those current at the time of your appointment, and are confirmed when you book. Our cancellation and rescheduling terms are set out in your booking confirmation; please give us as much notice as you can so the time can be offered to someone else.

Whether a service attracts a private health fund rebate, a Medicare benefit or insurer funding depends on your cover, your referral and your eligibility. We will tell you what we can, but we cannot guarantee what a third party will pay.

6. Links to other websites

We link to research articles, professional bodies, our co-located partners and our booking platform. Those sites are not under our control. A link is not an endorsement, and we are not responsible for their content, availability or privacy practices.

7. Content and copyright

The text, photographs, video, graphics and layout of this website belong to On Form Physiotherapy or are used under licence. You are welcome to read, print and share pages for your own personal use. Please do not reproduce, republish or adapt our content for commercial purposes without asking us first.

8. Availability

We aim to keep this website accurate and available, but we do not guarantee it will be uninterrupted or error-free, and we may change or remove content at any time.

9. Your rights under Australian Consumer Law

Nothing in these terms excludes, restricts or modifies any consumer guarantee, right or remedy you have under the Competition and Consumer Act 2010 (Cth), including the Australian Consumer Law, or under any other law that cannot lawfully be excluded.
