Frequently Asked Questions.

All your questions about getting started with On Form Physio, answered here.

In short, no. We’re primary contact practitioners, so you don’t need a doctor’s referral to be treated. In most cases, seeing a physiotherapist first means you can get started on your treatment sooner, without waiting on a GP appointment first.

You will need a referral if your injury is being paid for by another party — this includes WorkCover, Motor Vehicle Accident, Income Protection, or the Medicare GP Chronic Condition Management Plan (GPCCMP) Program. We’re unable to bill third-party payers without this information.

If you’re covered under one of these compensable programs but the referral or claim paperwork isn’t sorted yet, you don’t need to wait — you can start treatment by paying upfront and claiming the cost back from your insurer once everything’s been processed.

If you’ve seen a specialist or recently had surgery, it’s helpful if you can bring the most up-to-date correspondence from your doctor — this gives us the right information to guide your recovery while your surgical repair is still healing. We have good collaborative working relationships with specialists across Perth, and try to liaise with them to support your care.

No. There’s a significant gap between the Medicare rebate and the actual cost of running the practice, so we’re not able to bulk bill GPCCMP appointments.

If you’re eligible under the GPCCMP, you’ll be billed at our standard private rate at the time of your appointment. You can then claim your Medicare rebate back directly to your own bank account — this is processed after your appointment and usually takes 24–48 hours to come through.

The easiest and fastest way to book is online — find a day and time that suits you best. If you’d rather not deal with the booking system, fill out our contact form and we’ll get in touch with you directly.

Yes. We have facilities for you to claim using your private health insurance at the time of your appointment. Because rebate amounts vary so much between insurers and cover levels, we can’t estimate what you’ll get back — your insurer will be able to give you a clearer picture if you check with them directly.

Please allow up to 45 minutes for your first appointment, and up to 30 minutes for follow-up appointments. Your physiotherapist will let you know if they recommend a longer appointment, particularly if you need treatment across multiple areas.

Wear loose, comfortable clothing you can move freely in — your physiotherapist may need to see and assess the area you’re being treated for. If you have any relevant referral letters, X-rays, scans or reports from your doctor or specialist, bring these along too, as they help with your assessment.

Your physiotherapist will start by asking about your injury history, symptoms and goals, then move into a physical assessment relevant to your situation. From there, they’ll talk you through what they’ve found, and put together a plan for your treatment going forward — including what to expect from follow-up sessions and any exercises to work on in between.

This varies from person to person, depending on your injury, history and goals. Your physiotherapist will give you a clearer idea once they’ve assessed you, and will keep adjusting your treatment plan as you progress rather than locking you into a fixed number of sessions upfront.

We understand plans change, but because your appointment slot is reserved specifically for you, the following fees apply if you need to cancel or reschedule.

Standard Physiotherapy Appointments

  • 24 hours or more before your appointment — no charge
  • On the day of your appointment — 75% of the scheduled fee
  • Did not attend — 100% of the scheduled fee

These fees can be held as a credit on your file, to be used for another appointment within the following two weeks.

Bike Fitting Appointments

Because bike fitting sessions run longer and are in high demand, all bookings are tentatively held by providing your card details through our Halth booking system, or by paying a 50% deposit to confirm your appointment.

  • 48 hours or more before your appointment — no charge
  • Within 48 hours of your appointment — 50% of the scheduled fee
  • Did not attend — 100% of the scheduled fee

We understand that unforeseen circumstances come up — if something genuinely outside your control affects your ability to attend, let us know and we’ll always use discretion in applying this policy.

We have a couple of options if this is you. We offer a mobile service where we come to you — get in touch for more details on this.

If you’re beyond a reasonable travelling distance, we’re also happy to conduct an assessment using telehealth — a virtual way of still providing physiotherapy care. Get in touch with our team for more information on this option.

In that case, you’ll likely need an extended appointment time so we can give the right care and attention to each of your concerns. Let us know when booking and we can organise this for you.

We’re proud to be part of the local communities we live and work in, and we love partnering with local sporting groups. Send us a message and we can talk through what your team or organisation needs.

Get in touch and we’ll do our best to understand your needs and provide a quote for the service we’d recommend. This takes into account things like your private health cover, the complexity of your situation, and what type of treatment is likely to suit you.
